Health Check answers and results
The Health Check calculates a score in your browser. Simply completing it does not subscribe you to an email list. If you request an emailed copy, the form sends FBCC your name, email address, score, result category, and two selected starting points with their ratings. Those details are stored in the website’s WordPress database through WPForms and used to send the requested results. The results request form does not submit all twelve individual answers.
Optional newsletter
The separate newsletter form sends your name and email address to AWeber for the FBCC Family Business mailing list. It does not send your Health Check answers or results to AWeber. Subscribing is optional. You can unsubscribe using the link in an FBCC newsletter. AWeber processes subscriber information under its own privacy policy.
Contacting us
Email links open your own email application. You choose whether to send the message and what to include. We use information you send to respond to your request and discuss possible next steps. Please do not submit sensitive personal, clinical, financial, employee, or confidential business details through this website or ordinary email. Requesting information does not establish a counseling or consulting relationship.
Service providers and website activity
The site uses HostGator for hosting, WordPress and WPForms for website and form functions, email services to deliver results, and AWeber for optional subscriptions. These services may process technical information needed to operate the site, deliver messages, prevent spam, or maintain security. The site contains links to other websites; their practices are governed by their own notices.
We have prepared measurement signals for assessment completion, results requests, and contact clicks. These signals exclude names, email addresses, scores, and answers. A Google Analytics connection has not yet been verified for this site. We will update this notice if analytics or other data uses change.
Saved requests and your choices
Results requests currently remain in WordPress until they are manually removed. You may email us to ask about access, correction, or deletion of information you submitted. We may need to confirm your identity before fulfilling a request. Copies may also remain in email systems or routine hosting backups according to those services’ retention processes. Newsletter subscriptions can be managed separately through AWeber’s unsubscribe links.
